with 9 designs by Beverly Mayfield Sunshine.  No. 3083, Suzanne McNeill Design Originals, 1995, 11 pages.  Booklet in very good condition,  Only flaw is there are some indented marks on the front cover where someone used it as a pad for writing on a piece of paper on top of it.  Shows in the light when  booklet tipped.  See photos as part of the description.
